How to loot a treasure chest:
Target it. If you have trouble targeting it, type: /target treasure, or just /tar trea.
Open it. Type /open. You can also smash it for a long time, but opening it really is much smoother.
Loot it. If right-clicking doesn't work well, you can re-target and type /loot.
There are different types of treasure chests.
The two most important are "Regular Treasure Chests" and "Legendary Treasure Chests".
Treasure chests can generally only spawn when the target NPC is defeated in non-trivial instance.
Regular Treasure Chests come in different sizes.
Small (Tier 0-2), Normal (Tier 3-4), Mighty (Tier 5+)
Regular Treasure Chests come in different qualities. Chests of the same size have the same or very similar loot tables. However, chests of higher quality have a higher chance to drop loot, or drop the "good" loot.
Malachite (lowest), Jasper, Amber, Topaz, Sapphire, Ruby Diamond, Blue Diamond, Pink Diamond (highest)
For every class, there is one specific chest, which contains spells, tomes, Mythstones and other suitable items, for that class in addition to the regular loot of the chest.
A "Hard Mode Treasure Chest" always appears when defeating a boss NPC in (hard-/hard/hard+) mode. It contains an "Invaluable Coin", which increases faction with Token merchants in Plane of Knowledage. Hand the "Invaluable Coin" to Cloe or Walter. These chests drop both, in group- and in solo-instances (except Tier 1 solo-instances).
Legendary Treasure Chests are NPC specific. They generally drop 1 or 2 legendary items. Tier 7 and higher legendary treasure chests may also contain an additional random rare-drop-item, Tokens of Legendary Might or even a Reliquary.
Mighty Treasure Chests generally contain more items than regular legendary treasure chests and may also have additional items, such as rank 2 spells. They have a small chance to contain a mythical item.
Mythical Treasure Chests and Ancestral Treasure Chests contain items of the highest quality, which may include high rank spells and mythical items.
The size and quality of the treasure chest is defined by the following factors:
The event / NPC who spawns it. Tier 0 spawn Small Malachite Treasure Chests. In higher tiers, the event / NPC defines the base of the treasure chest. The higher the tier, the higher the base. Tier 1 and 2 can spawn different types of small treasure chests.
The difficulty mode of your instance. If you scroll down to Group-Instance Chests or Solo/Duo-Instance Chests, you find a table.

Various boss NPCs can spawn NPC-specific Legendary treasure chests which contain bonus loot, usually in the form of legendary items. These chests can also contain spells, tokens and reliquaries.
Legendary items are copies of "regular" items but come with a minimum of HP, Mana, Endurance, depending on their Tier (e.g. 140 for Tier 6) and an additional augmentation slot of type 6 for weapons or type 15 for other items. Legendary weapons often have improved proc rates (150 instead of 100%). Legendary items with a click-effect sometimes have the cast-time of the effect reduced.
Boss NPCs can have none or up to three different chests (Legendary, Mighty and Mythical/Ancestral). If a treasure chest appears, and which one it is, depends on the difficulty mode, the rarity of the chest and bit of luck.
Many chests have a rarity of "common", which means if the NPC is defeated in the difficulty mode "Normal" or above, a Legendary Treasure chest appears. If defeated in a lower mode, the chest may or may not appear.
When an NPC has access to multiple treasure chest types, there's a chance for the superior variant, the Mighty Treasure Chest, to spawn. This likelihood increases as the difficulty mode rises. Typically, a reasonable chance begins at Hard-, while Intense mode offers a significantly elevated probability of encountering it.
Rarities:
Rare: a chest (such as a legendary treasure chest) is guarnteed if the instance is in difficulty mode Intense; in lower modes it's more likely to not obtain a chest
Uncommon: a chest is guaranteed in Hard- and above.
Common: a chest is guaranteed at Normal and above.
Tier+: a chest is always appear from Normal- upwards; this rarity is called tier+, because it's often applied to NPCs who are one tier above the instance's tier (e.g. a tier 11 NPC in an instance that's classified as tier 10).
Tier++ chests are guaranteed from Easy+ and are usually tied to NPCs two tiers above.
Tier+++ chests are guaranteed from Easy and are usually tied to NPCs three tiers above.
